Forklift Trucks & Pallet Jacks Safety Checklist

• Are trailer wheels chocked when loading and unloading with fork trucks?
• Are employees working in the racks from elevated platforms wearing safety belts for fall protection?
• Are safety cages (secured to the mast) used when employees are being elevated by the forklifts?
• Are records maintained to substantiate operator training?
• Is a file maintained of daily inspections for each vehicle?
• Is a written operator-training program formulated?
• Are only authorized and trained operators permitted to operate powered industrial trucks?
• Are brakes operating properly?
• Are mirrors positioned at corners so drivers can see around corners?
• Is the equipment tagged out when defects are noted?
• Is equipment under a preventive maintenance program?
• Are battery-charging stations posted with “NO SMOKING” signs?
• Are emergency eyewash and drenching shower facilities provided at the battery-charging station?
• Is personal protective equipment (such as face shields) provided at each battery-charging station?
